Synopsis: This study will evaluate a skills-focused intervention designed to reduce sexual transmission of HIV in China. The program will target HIV infected former plasma donors (FPDs) and injection drug users (IDUs).
Details: Although China is experiencing a rapid spread of the HIV epidemic, prevention efforts have been limited. This project will evaluate an intervention designed to reduce transmission of HIV from two infected groups (FPDs and IDUs) to their partners. Participants will be assigned to either the intervention or a control condition. Participants in both groups will have ongoing access to optional HIV post-test counseling sessions, free high quality condoms, free HIV testing of partners, and free treatment from health care workers trained to treat sexually transmitted infections (STIs) and HIV-related infections. Participants in the intervention condition will participate in eight small-group sessions that will address reducing transmission acts, disclosure of serostatus and testing of partners, coping with stigma and health problems, knowledge of disease progression, and negotiation of health care. Monthly maintenance workshop boosters will be offered to provide social support for 18 months following the intervention. Primary outcome measures will include self-reports of sexual and drug or needle transmission acts, STI incidence, disclosure to and HIV testing of partners, and health-seeking behaviors. Outcome data will be collected at study entry, immediately post-intervention, and 6, 12, and 18 months after the intervention.

Protocol Entry Criteria: Inclusion Criteria - HIV infected - Spouse or fiancé(e) is HIV negative or of unknown HIV status - Participates in high risk transmission behaviors as assessed by questionnaire - Resident of selected study villages in China - Willing and able to take part in the trial and the follow-up
